Objective: To evaluate the prevalence of and predictors for underlying significant lesions in women with low-grade squamous intraepithelial lesion (LSIL) smears. Methods: Records were retrospectively reviewed for 208 women with LSIL who underwent colposcopy and histological evaluation from October 2004 through April 2009. Results: Mean age of the patients was 38.5years. Forty-four (21.2%) women were nulliparous; 20 (9.6%) women were postmenopausal; 29 (13.9%) women tested positive for HIV. Thirty-three (15.9%) women were current users of combined oral contraceptive pills. The pathological results of initial colposcopic evaluations were: 63 (30.3%) with cervical intraepithelial neoplasia (CIN) 2–3; 62 (29.8%) with CIN 1; 4 (1.9%) with cervical cancer;...

Objective: To examine the association between mental health problems among pregnant women and those in the postpartum period using a nationally representative sample of 6538 women aged 15–49years from the National Survey of Demography and Sexual and Reproductive Health in Paraguay. Methods: The predicted probabilities (PP) of common mental disorders (CMD) and suicidal ideation were assessed using the Self-Reporting Questionnaire (SRQ-20) and logistic regression models. Results: No evidence was found of an increased risk for mental health problems associated with being pregnant or in the postpartum period alone. The risk for CMD during pregnancy and the postpartum period and for suicidal ideation during pregnancy was...

Objective: To investigate whether the molecular classification of endometrial cancer based on gene expression profiles can predict the biological behavior of the tumors and inform prognosis. Methods: An array containing 492 genes was used to generate gene expression profiles from 35 tumor samples. A hierarchical cluster algorithm was used to compare gene expression patterns among the tumor samples. Results: A cluster analysis revealed 3 distinct tumor clusters. A comparative analysis of tumor type, grade, FIGO stage, and depth of myometrial invasion revealed significant differences in grade and stage among the clusters, which appear to group tumors with specific clinical behaviors. Moreover, the cluster analysis initially...

Objective: To analyze the trend in maternal mortality ratio (MMR), characteristics and causes of maternal deaths, and factors influencing the MMR in China between 1996 and 2005. Methods: Data used in this study are based on the nationwide maternal mortality surveillance system. Trend in MMR and cause-specific mortality were analyzed using the Cochran-Armitage trend test. Results: From 1996 to 2005, MMR was higher in rural areas than in urban areas; and highest in remote areas followed by inner lands and coastal regions. The overall MMR in China decreased from 64.1 per 100000 live births in 1996 to 47.6 per 100000 live births in 2005. MMR...
Prevention of unsafe abortion in countries of Central Eastern Europe and Central Asia

Despite permissive laws and a well-developed network of facilities, the incidence of unsafe abortion and the resulting maternal mortality is unacceptably high in Central and Eastern Europe and Central Asia, with one-quarter of all maternal deaths reported to occur as a consequence of abortion. Among the reasons that oblige women to submit themselves to risky procedures are new legislative barriers to accessing pregnancy termination, the unequal distribution of abortion services and healthcare providers, the increased costs of abortion services or unofficial payments, coupled with an increase in the number of impoverished and disadvantaged individuals. The quality of abortion services remains...
Minimal deviation adenocarcinoma of the uterine cervix

Background: Precise preoperative diagnosis of minimal deviation adenocarcinoma (MDA) of the uterine cervix is often difficult because the histological features of MDA closely resemble those of normal cervical glands. Objective: To review the developments in the diagnosis and treatment of MDA over the past 35years. Search strategy: We performed a meta-analysis of all case reports published in the English and Chinese languages between 1975 and 2009 that included a histopathologic diagnosis of MDA. Pooled odds ratios and 95% CIs were calculated for comparisons and analyzed. Results: The histopathologic diagnosis of MDA remains difficult and is currently based on antigen detection by immunohistochemistry. Cytologic assessment and routine biopsy...
Prevention of unsafe abortion: Analysis of the current situation and the task ahead

An evaluation of the current situation regarding implementation of the plans of action for the prevention of unsafe abortion shows that more than half of the countries selected for the initiative have progressed as planned or have surpassed expectations. The greatest barriers to faster progress have been insufficient resources or infrastructure and the sensitivity of the subject matter, as well as lack of a more active commitment from the societies of obstetrics and gynecology and their members. The most frequently mentioned facilitating factors were the strength of the project/FIGO leadership, the commitment of the collaborating agencies, and incorporation of the...

International collaboration with organizations and agencies is a basic requirement for the success of the FIGO Initiative for the Prevention of Unsafe Abortion and its Consequences. Many activities being carried out by the organizations form a part of the plans of action of all countries participating in the Initiative. It was, therefore, not difficult to obtain their collaboration in implementing the plans of action. The many ways in which they have collaborated and continue to do so are described in this article. This collaboration has saved time, avoided duplication of effort, and has also satisfied the Accra Agenda of Action...
Goals and activities proposed by countries in seven regions of the world toward prevention of unsafe abortion

This article describes the goals and activities included in the national plans of action for the prevention of unsafe abortion. With broad variability, all were in line with the initiative's purpose of contributing to reduce the number of women who have unplanned pregnancies and induced abortions, as well as the maternal mortality and morbidity associated with unsafe abortion. The interventions proposed can be classified in 5 groups: (1) to work toward better national policies through obtaining and disseminating accurate evidence-based data on abortion and to sensitize policy makers and health professionals on issues pertaining to unsafe abortion; (2) to reduce...
